New Patient Information
For your first visit to Pioneer Spine & Sports Physicians, please arrive 30 minutes early to allow time for registration and to complete a health questionnaire.
When you come for your appointment, please remember to bring the following:
Driver’s License or a valid photo ID
Insurance card
Referral letter (if required)
Reports, X-rays, MRIs, CT scans, or any other relevant information
List of medications (if any)
If you need to cancel an appointment: Please call our office during business hours and provide at least one day’s notice so we can offer your appointment time to other patients. If you cancel your appointment in less than 24 hours you will be charged a $20 cancellation fee.
We recognize that your time is valuable, and we make every effort to run on schedule. Occasionally, emergencies or patients needing extra time can cause delays beyond our control. We apologize for any inconvenience and appreciate your understanding.

How do I schedule an appointment?

Do I need a referral to be seen?
Referral requirements vary by insurance. Please check with your primary care provider to see if a referral is needed before scheduling.
What should I bring to my first appointment?
For your initial consultation, bring any required referral letters, insurance card, and photo identification. It’s also helpful to bring copies of relevant medical records such as X-rays, MRIs, CT scans, or reports, as well as a list of current medications. To make the most of your visit, consider writing down any questions you want to ask your doctor.
Is my medical information private?
Yes. Your medical records are handled with the highest level of confidentiality. Our staff is trained to protect your privacy and will not release any information without your consent.
What is the MEDENT Patient Portal?
The MEDENT Patient Portal gives you secure, online access to your medical records and an easy way to communicate with our office. Think of it like online banking—your information is private, and only you can access it.
With the portal, you can:
Send and receive non-urgent messages from your doctor’s office
View test results and educational materials
Update your personal information, including insurance and contact details
Request prescription refills
View past and upcoming appointments
Access and print your current medications and allergies
Sign Up: You must be a patient at our practice. Contact the office to receive your activation code, then create your username and password to access your portal account.
Security: Your health information is protected with encrypted communication, secure access codes, and personal login credentials. Only you can access your account.
Benefits: The portal helps you stay informed and take an active role in your healthcare. It reduces phone calls and paperwork, allows you to download or print records for other providers, and ensures important documents are safe and accessible online.
